Knot insertion

Given a curve \(\mathbf{C}(u)\) related to the vector \(\mathbf{U}\) and control points \(\mathbf{P}\), the algorithm knot_insert computes the new KnotVector \(\mathbf{V}\) and new control points \(\mathbf{Q}\)
